How well do you think that you know yourself? The Health Risk Assessment (HRA) is a multifaceted interactive tool offered annually at Your information is confidential.

Participants who complete the HRA enter information such as blood pressure, cholesterol and life-style behaviors.

When finished you receive a confidential Personal Wellness Report designed to help you understand potential lifestyle health risks, links with support information, and an action plan to enhance healthy life-style behaviors.

  • To enable you to objectively monitor your health over time.
  • To determine how ready you are to change certain behaviors.
  • To get an HRA completion award.

Complete your HRA in 5 easy steps:

Step 1: Go to

Step 2: Click on Civilian wellness or Active Duty wellness

Step 3: Log on with your user ID and password (if you have forgotten, click on 'Forgot ID/Password')

If this is your first time click on "Create Account" -- you will be asked some preliminary questions and then you will create your account by selecting your own login ID and password.

Step 4: Click on "Take the assessment now" and answer all of the questions (the HRA takes approximately 10 minutes).

Step 5: Print your Personal Wellness certificate.

Great job! You just took the first step to living a healthier life.

Bonus step: Take your Personal Wellness Certificate to your local CHPS office to claim your award (To be eligible for the award, HRA should be completed within 2012).
